South Coast School Sport have informed the District that ALL medical/permission booklets must be completed online and typed not handwritten. This booklet must be fully completed on line, printed and signed by a parent /guardian, then taken to school to be signed by the Principal.  This medical booklet must accompany your child and be handed in at the start of the trial.  (Handwritten booklets and unsigned booklets will result in your child not being able to trial.)   

Each student/parent must down load this booklet off the South Coast School website.

NB.

Booklets will be required at:

• Any  District Team Sport Trial.​
• Students who qualify for the Pacific District Swimming Team to compete at the Regional Trial. (Booklet taken to the Regional Swimming Carnival)
• Students who qualify for the Pacific District Cross Country Carnival as part of the Beenleigh Zone Team. (Booklet taken to the 10 to 19 Pacific District Cross Country)
• Students who qualify for the Pacific District Track and Field Carnival as part of the Beenleigh Zone Team. (Booklet taken to the 10 to 19 Pacific Track and Field Carnival)

 Booklets will not be required at:

• The Pacific District Swimming Carnival. (School team managers are responsibility for keeping medical and contacts details of their own students)
• At the Zone Divisional and Championship Athletics Days. (School team managers are responsibility for keeping medical and contacts details of their own students)
• At the Zone Cross Country Carnival. (Schools are responsibility for keeping medical and contacts details of their own students)
